Initial Steps:

-  Contact our Executive Director  
-  Submit New Group Application
-  Submit Municipal Resolution to Apply for Membership (Template here)

Upon Acceptance:
Please review the Consortium’s Municipal Cooperative Agreement and submit the following items through the Consortium's Upload Portal:

1.    A resolution appointing the municipality’s representative to the Consortium Board of Directors.  (Sample Resolution).
2.    Place on municipal letterhead and send the Excellus Broker Termination directly to Excellus. (Sample Here)
3.    Complete and Submit Online Enrollment selection
4.    Audit roster provided by the Consortium and complete member eligibility and dependent verification certification. 
5.    Notify Consortium of who will be the representative to the Joint Committee on Plan Structure and Design.
             * Each entity is allotted one voting Management seat and each Labor group is allotted one voting seat. 
6.    Upon Board approval of amended MCA, submit signed signature page (near year-end) (Signature Page).
